Exosome therapy is one of the newest and fastest-developing areas of regenerative medicine. Discovered in the 2010s as a byproduct of stem cell research, exosomes are today considered the “main actors” that largely provide the effects of stem cells. How Are Exosomes Produced?

1. Stem Cell Culture

Mesenchymal stem cells (from bone marrow, umbilical cord, or adipose tissue) are multiplied in the laboratory under GMP (Good Manufacturing Practice) standards.

2. Conditioned Medium Collection

The conditioned medium containing exosomes secreted by stem cells in the culture medium is collected.

3. Isolation and Purification

  • Ultracentrifugation: Most common method — precipitation at 100,000 × g
  • Size exclusion chromatography
  • Polymer precipitation kits

4. Characterization

  • Size analysis (nanoparticle tracking analysis)
  • Surface markers (CD9, CD63, CD81)

Human Clinical Data (As of 2026)

The number of published randomized controlled human studies is still extremely limited. Existing clinical reports are in the nature of small case series or observational studies. Although some private clinics offer the application as a routine service, this does not mean clinical approval.

Frequently Asked Questions

Is exosome therapy better than stem cell therapy? A definitive answer cannot be given. Some researchers argue that exosomes provide the beneficial effects of stem cells without carrying their risks. However, comparative clinical data is insufficient.

Is exosome therapy safe? The safety profile in animal studies is good. Side effect reports in humans are rare. However, long-term safety data is not yet available.

How many sessions are required? The standard protocol has not yet been defined. Most clinics perform 1–3 sessions; they monitor results.

When are results seen? In animal studies, effects were observed within 4–8 weeks. In human data, this period is reported as 6–12 weeks.

Is exosome therapy the same as hair transplantation? No. For hair, exosomes are applied to the scalp or hair follicles. For ED, they are injected into the corpora cavernosa. The mechanism is similar but the target tissue is different.
